We did that at the conference on "happiness at work" that we arranged
recently. Here's a pic of the process:

People arrived, signed in at the door and were then pointed to this
table. They could put whatever thay wanted on the name tages, and some
people got pretty creative. The whole thing worked great, even for a
conference people paid a lot of money (500$ for 1 day) to participate in.

Incidentally, this was only the first do-it-yourself exercise of the
day. We had many - even lunch was DIY :o)
